There&#8217;s nothing formal required here \u2013 just a few best practices and some key questions to answer.<\/p>\n<p>If you haven&#8217;t created one yet, think of all the forms you use now&#8230; Wouldn&#8217;t it be easier to have at least one of them online? Luckily, it can be \u2013 in minutes!<\/p>\n<h2><strong>Create Your First Form<\/strong><\/h2>\n<ol>\n<li>On the home page click the Create Project button and choose your project type.<br \/>\n<img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2021\/08\/WS24.png\"><\/li>\n<li>You have other options, but in this case, create your form by using &#8216;Start with a Blank Survey&#8217;.<br \/>\n<img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2021\/08\/WS25.png\"><\/li>\n<li>Next, provide the following details:<\/li>\n<p><img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2021\/10\/PT4.png\"><\/p>\n<ul>\n<li><strong>Name<\/strong>: Assign a unique name to differentiate your form from other projects.<\/li>\n<li><strong>Default Language<\/strong>: This is the language in which your form will be created. The primary language cannot be changed after questions are added to the form.<\/li>\n<li><strong>Add Languages<\/strong>: Allows you to create project in multiple languages other than the primary language selected. This is optional and can be set\/modified anytime from Options under the Design tab.<\/li>\n<li>Click on Create to build your first form.<\/li>\n<\/ul>\n<li>Drag and drop a Demographics question from the left-hand side panel.<\/li>\n<li>Based on the needs in your form, select from the a list of pre-defined fields under Demographics and click on Save.<\/li>\n<div class=\"sogo-help-info-box\"><i class=\"fal fa-info-circle\"><\/i><\/p>\n<p class=\"sogo-info-text\">\n<p>You may preview the question, if you wish, to review its display online. Use the &#8216;eye&#8217; icon in the top right of the screen to preview your form at any time.\n<\/p><\/div>\n<p style=\"padding-left: 60px;\">\n<li>You may also choose to drag and drop a <a href=\"\/help\/how-to-add-a-mutiple-text-box-question\/\">Multiple Text Box<\/a> question to ask any questions that require to participant to include multiple fields of details.<br \/>\n<img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2022\/08\/R30.2.166.png\"><\/li>\n<p><strong>Preview<\/strong> the question, as desired.<br \/>\n<img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2022\/08\/R30.2.167.png\"><\/p>\n<li>You can also add an <a href=\"\/help\/how-to-add-an-attachment-question\/\">Attachment question<\/a> for respondents to upload their information through files.<br \/>\n<img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2022\/08\/R30.2.168.png\"><\/li>\n<p><strong>Preview<\/strong> the question, if you like.<br \/>\n<img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2022\/08\/R30.2.169.png\"><\/p>\n<li>A <a href=\"\/help\/how-to-add-a-horizontal-radio-button-question\/\">Radio Button question<\/a> is a good choice for a final confirmation\/declaration statement.<\/li>\n<p><img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2022\/08\/R30.2.170.png\"><\/p>\n<div class=\"sogo-help-info-box\"><i class=\"fal fa-info-circle\"><\/i><\/p>\n<p class=\"sogo-info-text\">\n<p>Any questions that are critical can be marked as mandatory.
