After all, you probably need to use that contact information \u2013 email address, mailing address, phone number \u2013 and if the data you collect isn\u2019t even in the right format, you\u2019re out of luck. In this case, the simple fields and automatic validation of the Demographic question type may be perfect for you. <\/p>\n<p><img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2021\/03\/QT13.png\"> <\/p>\n<p>To add a Demographic question to your survey, drag and drop the question type from the panel on the left to the correct spot on the right.  <\/p>\n<p><strong>This question includes pre-formatted fields that automatically validate the data received, such as: <\/strong><\/p>\n<p><img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2022\/08\/R30.2.37.png\"> <\/p>\n<ul>\n<li><strong>Name:<\/strong> Choose a single field for Full Name or separate fields for First Name, Middle Name, and Last Name.<\/li>\n<li><strong>Gender<\/strong> <\/li>\n<li><strong>Date of Birth:<\/strong> Customize the date format by clicking on the settings icon. If it\u2019s helpful, you can even set a default date to save participants&#8217; time. <\/li>\n<li><strong>Address:<\/strong> Choose between US and international address formats and select the fields you\u2019d like to include. <\/li>\n<li><strong>Telephone\/Fax:<\/strong> Choose between US and international address formats and select the fields you\u2019d like to include.  <\/li>\n<li><strong>Email address:<\/strong> As needed, choose to require participants to re-enter email address. <\/ul>\n<\/li>\n<p>Once you have added the fields, drag and rearrange their order in the display on the right. <\/p>\n<p><img decoding=\"async\" src=\"\/help\/wp-content\/uploads\/2022\/08\/R30.2.38.gif\"> <\/p>\n<p>Once your fields are added, you may top choose to customize the default labels.
